(1). CORRESPONDING INVOICE NUMBER
(2). ITEM NUMBER (PART NUMBER) AND QUANTITY
(3). INFORMATIVE DESCRIPTION OF REASON FOR RETURN

2. SPI RMA DEPT. WILL FAX YOU A RMA REQUEST FORM IMMEDIATELY UPON YOUR REQUEST

3. COMPLETE THE RMA REQUEST FORM AND FAX BACK TO SPI WITH INVOICE COPY. INCOMPLETENESS WOULD RESULT IN A DELAY OF RMA PROCESS

4. ALLOW 3 HOURS TO PROCESS YOUR RMA REQUEST. THEREUPON, A CONFIRMATION OF RMA NUMBER & WHAT DEFECTIVE PARTS YOU SCHEDULE SEND BACK WILL BE INFORMED VIA PHONE

5. THE RMA NUMBER IS VALID FOR FIFTEEN DAYS FROM THE DATE ISSUED. PLEASE SHIP YOUR RETURN ITEMS PROMPTLY VIA FREIGHT PREPAID

6. DISPLAY THE RMA NUMBER PROMINENTLY ON THE SHIPPING LABEL OR ON THE EXTERIOR SHIPPING CONTAINER. W/O RMA NUMBER WILL BE REJECTED OR SEND BACK FREIGHT COLLECT

7. PRODUCTS WILL BE SHIPPED BACK TO YOU VIA UPS/TRUCKING GROUND ONLY. FOR QUICKER DELIVERY, THE FREIGHT DIFFERENCE WILL BE CHARGED AT YOUR EXPENSE

8. RMA DEPT. WILL NOT GIVE OUT CREDIT EXCEPT WITH THE APPROVAL OF OUR SALES AND ACCOUNTING MANAGER

9. RETURN PRODUCTS MUST BE SENT IN SECURE PACKAGING TO PREVENT ANY SHIPPING DAMAGE. ANY DAMAGES TO THE RETURN PRODUCT, YOU WILL BE INFORMED AND RETURNED FREIGHT COLLECT

10. THE RETURN PRODUCTS WITH DIFFERENT RMA NUMBER MUST BE PUT IN SEPARATE BOX RATHER THAN COMBINED IN ONE BIG CONTAINER. IF YOU HAVE TO COMBINE ALL ITEMS IN ONE CONTAINER, YOU MUST LABEL THE RMA NUMBER FOR EACH ITEM. IF THE PRODUCTS CAN NOT BE MATCHED WITH ATTACHED RMA NUMBER, IT WILL BE RETURN FREIGHT COLLECT

11. DOA PROGRAM COVERAGE -- 60 DAYS FROM THE DATE OF PURCHASE

12. RMA PROGRAM COVERAGE -- 1 YEAR PARTS AND LABOR FROM THE DATE OF PURCHASE

13. ALL RETURNED ITEMS MUST BE IN SALABLE CONDITION, ACCOMPANIED WITH ORIGINAL CARTON, PACKAGING MATERIALS, AND ALL ACCESSORIES

14. ANY PRODUCT WHICH HAS BEEN COSMETICALLY ALTERED IN ANY WAY OR FORM (I.E. PRIVATE LABEL, TAG, STENCILED, TAMPERED WITH, ETC.) WILL NOT BE ACCEPTED FOR RETURN FOR CREDIT OR REPLACEMENT

15. ANY DAMAGE CAUSED BY TRANSPORTATION SHOULD BE CLAIMED TO CARRIER RATHER THAN SPI

16. A 15% RESTOCKING FEE WILL BE ASSESSED IF THE REASONS FOR RETURN ARE SIMILAR TO THE FOLLOWING:
(1). YOU HAVE ORDERED THE WRONG ITEMS
(2). YOU ARE NOT SATISFIED WITH THE PERFORMANCE BUT NOT DEFECTIVE
(3). THE ITEMS YOU HAVE ORDERED ARE NOT COMPATIBLE WITH THE OTHER COMPONENTS
(4). ALL PRODUCTS RETURNED FOR CREDIT AFTER 30 DAYS TO THE PURCHASE DAY (INCLUDE THE EVALUATION ITEMS)

17. FREIGHT (SHIPPING) CHARGES ARE NON-REFUNDABLE AT ALL ITEMS

18. IF EACH TIME YOU MEET A SPECIAL SITUATION OR HAVE ANY DIFFICULTIES TO FOLLOW THE ABOVE RMA POLICY, PLEASE DON'T SEND BACK ANY ITEMS BEFORE YOU CONTACT OUR RMA DEPT.
